INFORMATION COLLECTED ABOUT YOU

Confidentiality

Catalyst Sport Massage Inc. respects customer privacy and the privacy of those accessing our website. We undertake to protect the confidentiality of our customers and users including all personal information supplied in the course of registering for events with us. Under no circumstances do we sell your personal information to third parties for commercial or marketing purposes.

Data Collected Automatically

We track certain generic information with respect to customers' use of our website. Information may include number of unique visitors, the number of times they visit, and the areas on the website that they favour. We only use this type of data on a collective basis, in summary form, rather than on an individual basis. This data is used for click stream analysis to help better understand site usage, so that we can improve our service to you. We do not link information automatically logged by such means with personal data about specific individuals.

IP Addresses

We track traffic to our website by collecting visitors' domain and IP addresses automatically; this information does not identify you as an individual, but only the computer that is being used to view the site. An IP address is a unique number assigned to your computer when you are using your browser on the Internet. We log this information so that we can monitor things like the number of visitors visiting the website; however, we do not use or link your IP address to any personally identifying information.

It is our intention to always allow you to opt out of any of our lists. When you receive any newsletter or email and you want to unsubscribe, you can simply click the instant "unsubscribe" links at the top of each message.

Information Gathered from Members

If you elect to purchase services at www.catalystsportmassage.com, you will be required to provide us with certain personal information, including your name, e-mail address, credit card number, expiration date, and billing address (this required information is subject to change from time to time). There may also be opportunities for you to provide us with additional information regarding your preferences and interests; however, is not required and is completely optional on your part. We may appoint third party organizations to assist us in providing you with the information, goods, products and services you request. In these circumstances, we will take steps to ensure that your personal information is held in strict accordance with our privacy policy and is only used for the purpose of fulfilling your request.

SECURITY OF PERSONAL INFORMATION

When you purchase treatment packages or access your payment information, we use an advanced encryption technology known as Secure Socket Layer (SSL) to protect the storage and transfer of your data from unauthorized parties. While on a secure page, such as our registration form, a lock or key icon appears on the bottom of your web browser. In addition to our online security efforts, we take reasonable steps to protect your personal data offline, as well. All of your information, not just the sensitive information mentioned above, is restricted in our offices. Catalyst Sport Massage Inc.'s main servers are operated from a dedicated Internet hosting facility with state-of-the-art physical security features.

CHANGES TO THE PRIVACY POLICY

Catalyst Sport Massage Inc. will occasionally update this Privacy Policy in response to changing business circumstances and legal developments. The date at the top of this page will change to reflect the implementation date of the most recent updates to the policy.

Please understand that when you purchase a treatment from Catalyst Sport Massage Inc., you are purchasing our time which is limited to certain hours during an event. Catalyst Sport Massage Inc. has implemented the following Cancellation & No Show Policy for our mutual benefit. Please review this policy and keep it in mind when booking your time slot(s) for treatment during your event.

I, the Purchaser, understand that in the circumstance that the event is canceled, Catalyst Sport Massage Inc. will make every effort to 100% refund my payment within 10 days of the event being cancelled.

I, the Purchaser, understand that in the circumstance that I incur any illness or injury prior to the event that prevents my participation in the event, Catalyst Sport Massage Inc. will refund 50% of my payment upon receipt of a valid physician's statement.

I, the Purchaser, understand that in the circumstance that I incur any personal bodily injury as a result of participating in the event and that injury prevents me from receiving my scheduled therapy treatment, my payment may be refunded in part at the discretion of Catalyst Sport Massage Inc.

I, the Purchaser, understand that if I miss my scheduled treatment session during the event for any reason other than injury as outlined above, my time slot will be forfeited and payment will not be refunded by Catalyst Sport Massage Inc.
